Thank you for considering Valley Health Plan! 
If you are interested in choosing Valley Health Plan and would like to keep your current doctor, contact us today! Let us help you make a decision about your care. Simply submit our online interest form or give us a call at 408.885.4760 or 1.888.421.8444 (toll-free). 

For the hearing and speech impaired, call the California Relay Service (CRS) by simply dialing 711 or the 800 CRS number of your modality.  You can also submit any question or concerns electronically to Member Services by using our Contact Us form.

If you do not see the provider you are searching for in our online Provider Search, please contact us.  Valley Health Plan is continuing to expand our networks and services - let us help you find the right care for you!

Valley Health Plan believes that it's your health, your choice.  We offer many provider networks for you to choose the right care for you and your family.  We also offer free health education and fitness classes to our Members including Zumba, yoga, healthy cooking demonstrations, and more!  At Valley Health Plan, we understand that alternative and complementary care is important - so we offer acupuncture and chiropractic services for a low co-pay.  Call us at 408.885.4760 or 1.888.421.8444 (toll-free) to speak to a Member Advocate to find out more about what VHP can do for you!

We encourage you and your family to choose a PCP that best fits your needs. We will make every effort to assign you to the PCP of your choice, however, if this is not possible, we will contact you to make another selection. If you do not select a PCP, we will assign one to you and your enrolled Dependents.  To find a PCP, you can use our online Provider Search or call Member Services at 408.884.4760 or 1.888.421.8444 (toll-free).
