We will make every effort to assign you to the PCP of your choice, however, if this is not possible, we will contact you to make another selection. If you do not select a PCP, we will assign one to you and your enrolled Dependents. If you are a resident of Alameda, San Mateo or Santa Cruz County, and are interested in choosing a PAMF Primary Care Provider located in those counties, please contact VHP Member Services at 408.885.4760 or 1.888.421.8444 (toll-free). Our member advocates can verify if your PCP is contracted with VHP in those counties. Except in the case of an emergency, all hospital services must be authorized by the Plan. Currently, VHP is contracted with the following hospitals:
• Lucile Packard Children’s Hospital • O’Connor Hospital • Saint Louise Regional Hospital • Santa Clara Valley Medical Center • Stanford Hospital Currently, there are 12 urgent care clinics, many with services available on a walk-in basis, available to VHP Members. Laboratory services are ordered by your physician when medically necessary. VHP is contracted with Quest Diagnostic Laboratory Services offering more than 400 Quest Patient Service Centers (PSC) in California – with 25 convenient locations throughout the County. Some physician clinics/offices offer onsite laboratory services. VHP Members enjoy pharmacy benefits and services offered for a $0 copayment. Members can fill prescriptions nationwide at Safeway and Walgreens pharmacy locations as well as select local pharmacies in Santa Clara County.
