Effective November 1, 2011, Navitus Health Solutions, a pharmacy benefit manager, will be managing Valley Health Plan’s (VHP) prescription drug benefit. You will have access to an expanded Pharmacy Network with even more conveniently located pharmacies in and out of Santa Clara County. As a Member, you will continue to enjoy a $0 co-pay prescription benefit, and can fill your prescriptions at the following VHP Plan Pharmacy locations:

• Safeway
  – All U.S. Locations
  
• Walgreens
  – All U.S. Locations
  
• Gardner Family Health Network (GFHN)
  – Gardner South County
  – St. James Health Center
  
• Palo Alto Medical Foundation (PAMF)
  – Mountain View Center
  – Palo Alto Center
  
• Customized Plan Pharmacy Partners
  – Navitus Specialty Rx (Diplomat)
  – Wellpartner (Optional Mail-Order Partner)

*Important Note:  If you are receiving care from a provider at Santa Clara Valley Medical  Center (SCVMC), Valley Specialty Center, or a Valley Health Center, you can continue to use the Plan Pharmacies at SCVMC. These Plan Pharmacies include: Valley Health Centers @ Bascom, East Valley, Gilroy, Lenzen, Milpitas, Moorpark, Sunnyvale, Tully and Valley Specialty Center.

There will be a 6-month transition period to continue coverage for all current prescription drugs (formulary and non-formulary). After the initial 3 months, Navitus Health Solutions will contact Members on non-formulary prescription drug(s) by mail to offer a covered alternative.

Refill prescriptions can be transferred by having your new Plan Pharmacy call the SCVMC Pharmacy Information Center at 408.793.2750. For Members with a non-SCVMC Plan Provider, the SCVMC Plan Pharmacies will no longer refill your prescription(s) after December 31, 2011.

You will receive a new VHP ID card that includes your pharmacy information in the mail. You must show your new VHP ID card when filling prescriptions at your Plan Pharmacy from this date forward.
