- Choose a Plan Pharmacy where you want to have your prescription filled.
Note: The SCVMC Plan Pharmacies are for VHP Members receiving care and with a prescription from a provider at Santa Clara Valley Medical Center (SCVMC), Valley Specialty Center & Valley Health Centers ONLY.
- Bring your VHP ID card each time you fill a prescription at a Plan Pharmacy. The card has information needed to fill your prescription.
Mail Order Options
- For short-term medications
Select Plan Pharmacies offer mail order service for filling prescriptions. Check with your Plan Pharmacy for additional service options.
Note: Valley Health Plan members have $0 co-pay for the prescription drug benefit. VHP does not cover charges for additional convenience services that Plan Pharmacies may offer.
- For long-term maintenance medications

Check with your Plan Pharmacy for mail order prescription refill options.
- Option 2
Navitus Health Solution’s mail order pharmacy service, Wellpartner, offers an easy way for you to get up to a 90-day supply of your maintenance medication for your chronic illness.
- Step 1: Enroll
Complete the mail order form available here, by registering online at www.wellpartner.com or by calling Wellpartner at 1.877.935.5797 (toll-free). Payment is not required for Formulary Prescriptions or prescriptions pre-approved through Valley Health Plan.
- To register online at www.wellpartner.com:
• At the top righthand corner, click "Wellpartner Online Pharmacy"
• Then, click "New Member (Sign Up)" to begin your registration.
- Step 2: Fill Your Prescription
Please make sure your VHP ID number is written on each prescription. Your prescription can be sent to Wellpartner using the following options:
- Your Plan Provider can give your prescription directly by:
• Calling 1.877.935.5797 (toll-free)
• e-Prescribe
• Faxing to 1.866.624.5797
- You can mail the original prescription with your order form to:

Please do not send prescriptions or have your doctor fax prescriptions to Wellpartner until you want them filled. Unless you notify Wellpartner differently, your prescriptions will be filled for the quantities prescribed by your doctor and allowed by your prescription plan benefit.
