Please contact Member Services at 408.885.4760 before making an appointment to verify that the provider continues to be part of the network. Valley Health Plan cannot guarantee that a doctor or facility will be a participating provider for the entire length of your enrollment.

Data on the page for name, gender, specialty, practice locations (medical group affiliations), languages spoken, and hospital affiliations is submitted by the provider or medical group during the credentialing process. It is the provider's responsibility to notify Valley Health Plan if there is a change in their information.  Some information may not be current as changes may occur at any time and the provider may not notify Valley Health Plan in a timely fashion.
