Welcome to Valley Health Plan!

As a new member, you’ll receive your VHP Identification (ID) card and New Member packet in the mail.  The New Member packet will include:

You may need to access health care services as soon as your coverage becomes effective.  If you are not sure of your effective date with Valley Health Plan, please ask your employer. 

If your health coverage is effective and you need care before you receive your permanent VHP Identification (ID) card in the mail, you can use your temporary ID card to access services. 

Nurse advice is also available to VHP members by calling the 24/7 Medical Advice Line at 1.866.682.9492.

Take advantage of what Valley Health Plan has to offer in just three easy steps! 

  1. Call Member Services, Monday through Friday between 9am-5pm at 408.885.4760 or 1.888.421.8444 (toll-free) to request your:
    • Identification number;
    • Network name; and
    • Primary Care Physician (PCP) name
  2. Fill in the empty fields on your temporary VHP ID card for reference.
  3. Access health care services as you need them.
    Doctor’s Visit – Call to make an appointment to see your PCP or mental health provider.
    Pharmacy – You will need the RxGroup#, RxBIN#, and RxPCN# information located on the bottom of your VHP ID card for pharmacy services. If the pharmacy system does not show you as a VHP Member, ask the pharmacist to call Navitus Customer Care at 1.866.333.2757 to verify your eligibility.
    Urgent Care – For care needed within 24-48 hours, go to a VHP Network walk-in Urgent Care clinic.
    Emergency Room – In the event of an emergency, call 9-1-1 or seek care at the closest hospital.
