VHP’s expanded network of doctors makes it easier to find the type of medical setting best for you. Our network includes PCPs located in small offices to large multiple office facilities. Our providers speak your language and can address your health care needs improving the overall health status of our community.
Valley Health Plan encourages you and your family members to choose your own Primary Care Physician (PCP) who is responsible for taking care of your routine health care needs. Your PCP will play a key role in coordinating your health care services including making referrals for medically necessary services.
As a Member, you have the right to change your PCP at any time. The change will take effect on the first day of the next month after your request is received. During that time, a new card will be sent to you with the name of your new PCP. You are eligible to be a VHP Member if you are an In-Home Supportive Services (IHSS) worker, or an employee of Santa Clara County, Valley Transportation Authority, Valley Medical Center Foundation, South Bascom Pediatrics, Inc., or the Council on Aging.
