Pediatric Asthma Disease Management Program

The Pediatric Asthma Disease Management Program is designed to enhance the care of pediatric patients, 0-18 years of age, with very mild to very severe asthma. The focus of the asthma education program is to teach patients and family members how to monitor their asthma, control symptoms and prevent flare-ups.

Patients and family members will receive a one-on-one teaching session. After completing the session, the patient and family will receive an asthma action plan with specific information on the patient’s needs. To help support good asthma management, families will receive information by mailings and phone calls from the program staff.
A doctor’s referral is needed to enroll in this program.​

  • Pre-registration through VHP is required unless noted otherwise.

  • Classes and schedules are subject to change or cancellation at any time without notice.

  • A Waiver of Liability form must be signed to participate in any exercise or fitness class offered by VHP.
