Combined Evidence of Coverage (EOC)

We are continuing to grow and expand our network to bring our Members more choices for care! 

The Benefits and Coverage Handbook, also known as the Combined Evidence of Coverage (EOC) has information about how Valley Health Plan(VHP) members get health services, the benefits available to members, and a member's healthcare rights.  

Valley Health Plan must get approval of the Handbook from our regulatory agency each year. The Handbook for our Covered California and IFP (Individual & Family Plan) has been approved for 2022. The 2022 Handbook for the Group Plan has not yet been approved. It will be posted once VHP gets approval. ​​

VHP got approval to increase our service area for the Individual & Family Plan (IFP) product in 4 zip codes in San Benito County on October 28, 2019. These zip codes are: 95045, 95023, 95024, 95004 which include the cities of Hollister, Aromas and San Juan Bautista. Valley Health Plan (VHP) also got approval on October 30, 2020 to offer our Individual & Family Plan (IFP) plans to people living in 12 zip codes in Monterey County. These zip codes are: 93901, 93902, 93905, 93906, 93907, 93908, 93912, 93915, 93933, 93955, 93962 and 95012 which include the communities of and around, Castroville, Marina, Prunedale, Salinas, Seaside, and Spreckels. VHP has also been expanding our provider network in Santa Clara, San Benito, and Monterey Counties. All IFP members, regardless of where they live have access to the entire provider network listed in our Provider Directory.

IFP members can look at VHP’s Provider Directory at https://eservices.sccgov.org/vhpservices. To look for a specific provider, select the “Individual & Family Plan product” under the “Product Type.”​
