Transitional Care Services (TCS) Program

Care transitions happen as your health condition and care needs change. The VHP Transitional Care Services (TCS) Program is here to help you as you move from one level of care to another. Phone support by a dedicated care team is offered in the first month as you transition into the community.

Services may include:

Staff will help you to schedule a follow-up visit with the primary care provider ideally within 14 days of discharge.
Staff will support care gap closure by scheduling additional visits, if needed.
Staff will coordinate other specialty appointments, if needed.
Staff will connect you to any necessary outside resources. This may include referrals to Enhanced Care Management, or continuing to Complex Case Management services.

Our team of care coordinators, social workers and registered nurses are ready to help you.
