  • Email VHP Broker Relations with the following information and we will send you an appointment packet via DocuSign. 
  • ​CA Department of Insurance License number 
  • Your name or Agency name 
  • Email address to send DocuSign to 

Your appointment effective date will be the first of the month of the quarter in which you requested the appointment. You can enroll your clients in VHP while you are waiting for your appointment to be processed.
