Care Management Program
Valley Health Plan (VHP) has a care management program designed to work with our members who have one or more health conditions to help improve their health and quality of life. The programs consist of case management, care coordination, and preventive health education and are FREE for members who qualify based on medical need. This program is voluntary and you can opt out at any time. Ways to enroll in the program: Your doctor may refer you to the program, Valley Health Plan may refer your to the program, and you can self-refer by calling the number below.

Staff members are available between 9:00 am and 5:00 pm (PST) Monday through Friday. If you use a TTY, call 1.800.735.2929 (toll-free) or your local Telecommunications Relay Service to talk to a program nurse.

Welcome to VHPs Pregnancy and Postpartum Wellness Program! We know that being a mom and mom-to-be comes with joy and challenges. At VHP, we care and value the health and wellness of all our moms, so we have designed this program to support you during this important time.
What is Maternal Mental Health?
Maternal mental health refers to the emotional well-being of moms during pregnancy and after giving birth. It is normal to experience a range of feelings during this time, like happiness, excitement, and sometimes stress or sadness. The program goal is to help all moms cope with these emotions and provide support and education.
Why is Maternal Mental Health Important?
Being a mom can be rewarding and overwhelming, and taking care of your mental health is important. When moms feel good emotionally, it benefits them, their babies, and their families. The program is here to help you stay strong and positive as you navigate motherhood.
What does the Pregnancy and Postpartum Wellness Program offer?
The program gives you the tools to get healthy and stay healthy. As part of the program, we offer to all our moms the following resources.
1. Support: a safe space for you to express your feelings and concerns. We provide access to trained professionals experienced in maternal mental health. They listen, offer guidance, and connect you to resources to help you navigate this important stage of life.
2. Education: easy-to-read materials to help you learn more about maternal mental health, self-care, and coping skills.
3. Referral: to connect you with local mental health professionals, therapists, or support services specializing in maternal mental health. We want to ensure every mom can access the appropriate care and resources to address their needs.
How do I Join the VHP Pregnancy and Postpartum Wellness Program?
If you are a VHP member, you are eligible to enroll at no cost to you. If you are not a VHP member, contact VHPs Member Services team to join today!

Transitional Care Services (TCS) Program
Care transitions happen as your health condition and care needs change. The VHP Transitional Care Services (TCS) Program is here to help you as you move from one level of care to another. Phone support by a dedicated care team is offered in the first month as you transition into the community.
Services may include:
- Staff will help you to schedule a follow-up visit with the primary care provider ideally within 14 days of discharge.
- Staff will support care gap closure by scheduling additional visits, if needed.
- Staff will coordinate other specialty appointments, if needed.
- Staff will connect you to any necessary outside resources. This may include referrals to Enhanced Care Management, or continuing to Complex Case Management services.
