VHP Care Management Program
VHP offers a wide range of care management programs to our Members. These programs are designed to help Members achieve a higher quality of life and health through education, coordination, connection to resources and improved communication between Members and their Providers. VHP's care management programs are supported by our 24/7 nurse triage and advice line.
The dedicated VHP Care Management Team provides services and education through case management, condition management, or care coordination. The team is staffed by:
- Registered Nurses
- Medical Social Workers
- Care Coordinators
- Community Outreach Specialists.
Shared decision-making (SDM) aids
Shared decision-making (SDM) aids provide information about treatment options and outcomes. SDM aids are designed to complement practitioner counseling, not replace it. SDM aids facilitate member and practitioner discussion on treatment decisions (NCQA, 2024).
Shared decision-making aids are offered to providers supporting VHP members to help achieve population health management goals.
VHP’s intent for the generation of shared decision-making aids is to research available evidence to obtain them for use by providers. These tools are reviewed by VHP clinical staff on a regular basis for updated versions, and to provide other shared-decision making aids to support providers in meeting their own population health goals.
