All IFP applications must include:

  1. Photo ID
  2. Proof of Address

  • Driver’s License (including AB60)
  • CA State ID
  • Passport (U.S. or foreign)
  • Work or school ID
  • Permanent Resident Card
  • International DL
  • Military ID

Proof of Address

  • Home utility bill
  • Cell phone or cable bill
  • Mortgage bill
  • California property tax bill
  • Rental/lease agreement
  • Deed/title to residential home
  • State or Federal tax returns
  • California vehicle registration
  • U.S. Postal Service change of address confirmation (form CNL107)
  • Voter registration confirmation
  • Insurance document (e.g. life, home, rental, car,)
  • Medical document
  • Employment document
  • Bank statement
  • Credit card statement
  • Letter of residency attestation
