Specialists

Verity Medical Foundation patients will be referred by their Primary Care Physician or other approved VHP Plan Provider to see a Network Specialist. Once approved, you will be given information on how to schedule an appointment with the specialist.

Laboratory

Verity Medical Foundation patients are covered within VHP Network Laboratories.

Pharmacy

Verity Medical Foundation patients are covered within VHP Plan Pharmacy Network.

Urgent Care Locations

Emergency Care
