Your Member ID Card

How can I request a new member ID Card?

If you lost your member ID card, your ID card is damaged, or if you find a mistake on your member ID card, contact VHP Member Services (toll-free) 1.888.421.8444 or by email at [email protected] to request a new card. 

Sign in to your VHP Connect account to view your member ID card online.  You can download the VHP Connect app for free on the App Store® or on Google Play®.

I am a new member and just received my member ID card.  What should I do now?

When your member ID card arrives, you should check the spelling of your name, date of birth, and Primary Care Provider (PCP).  If a PCP was selected for you by VHP, you still have the option to choose another PCP from our network.  Visit www.valleyhealthplan.org to use the provider search tool.  You may also contact VHP Member Services (toll-free) 1.888.421.8444 or by email at [email protected] to request help with PCP selection.
